Health and social care organisations in Derbyshire have been working closely together for some time, to improve care and services for people and make them as efficient and effective as possible.
An exciting project is underway to develop a shared care record for people accessing health and care services across Derbyshire. Partners across Joined Up Care Derbyshire are in the process of recruiting a technical agency to help us with our ambition of developing a single accessible electronic record for patients and service users.
Historically we have never quite managed to achieve this, but now we have a collective ‘joined-up’ vision of a record that can be easily/securely accessed by clinicians and others involved in a person’s care. The development also needs to greatly improve our system-wide analytical capabilities and understanding of population health data, removing any constraints previously fed by organisational boundaries.
Clearly, this is a significant project and our first major milestone is an initial ‘proof of concept’ by the end of March 2021. A system-wide group is currently meeting weekly to focus our efforts. We will provide regular updates as we move through this exciting and significant enhancement for the people of Derbyshire.
