1. Please could you supply the name, email address and telephone number of the commissioner with responsibility for packages of homecare for adults aged 18-64.
2. Please provide the number of adults aged 18+ in receipt of CHC funded care packages at year-end 2016/17, 2017/18, 2018/19, 2019/20, 2020/21 and 2021/22.
3. Of those adults in receipt of CHC funded care packages (q.2), please provide the number that were homecare packages for adults aged 18+ at year-end 2016/17, 2017/18, 2018/19, 2019/20, 2020/21 and 2021/22.
4. Of those adults aged 18+ in receipt of CHC funded homecare packages (q.3), please provide the number that were processed through the fast-track system (this is often related to end-of-life care packages) 2016/17, 2017/18, 2018/19, 2019/20, 2020/21 and 2021/22.
5. Of those adults aged 18+ in receipt of CHC funded care packages (q.2), please provide the number that are in receipt of personal health budgets 2016/17, 2017/18, 2018/19, 2019/20, 2020/21 and 2021/22
6. Please provide the ICB’s total gross expenditure on continuing healthcare funded packages for adults aged 18+ during the year for 2016/17, 2017/18, 2018/19, 2019/20, 2020/21 and 2021/22.

Please note that some of the scope of your request covers the timeframe prior to the merger of the former 4 Derbyshire CCGs into Derby and Derbyshire CCG in April 2019. The information for all these 4 CCGs is included.
Please also note that no figures for 2020/21 are provided; due to issues arising from the Covid-19 pandemic the way in which this information is held was temporarily changed during this year. As a result we are not able to break down the information that we hold to a sufficient level of detail to accurately respond to your questions with regards to this financial year.
