1. Does the ICB commission, provide or manage any or a combination of these type of care services provided in peoples’ homes?
– A domiciliary care bridging service for hospital patients that are fit to leave hospital but need care services at home – this service would typically be provided for a short period of time until the patient’s local authority can provide a more permanent service if needed
– Re-enablement services for hospital patients discharged into the community so as to get them fit and healthy to look after themselves
– A long-term domiciliary care service

1. Does the ICB commission, provide or manage any or a combination of these type of care services provided in peoples’ homes?
a. A domiciliary care bridging service for hospital patients that are fit to leave hospital but need care services at home – this service would typically be provided for a short period of time until the patient’s local authority can provide a more permanent service if needed
I can confirm that University Hospitals Derby and Burton NHS Foundation Trust (UHDB) and Chesterfield Royal Hospital NHS Foundation Trust (CRHFT) have a contract with a bridging service to support discharge to support with patient flow out of the acute hospital.
b. Re-enablement services for hospital patients discharged into the community so as to get them fit and healthy to look after themselves
These services are all provided within the ICB as part of the discharge process. Both the local authorities and Derbyshire Community Health Services (DCHS) provide reablement services to support discharge.
c. A long-term domiciliary care service
Yes for adults and Children and Young People who are eligible for Continuing Healthcare (including Fast Track) and Continuing Care (including Fast Track); this service is Transforming Care Programme funded; Retraction and Dowry funded or joint funded between health and local authority such as S117.
2. For any service provided:
a. What is the name of the service?
Any Qualified Provider (AQP) Homecare framework (for Adults CHC and Fast Track). All other placements are spot purchased currently.
b. What is the name of the organisation that provides the service?
There are 120 providers qualified on the AQP framework.
c. How many individuals are provided the service? Please be specific about the time periods that the answers relate to.
Bridging service – from 14 December 2021 to 13 December 2022: 477 patients.
Reablement service – from 14 December 21 – 13 December 2022: We only hold the number of patients discharged on Pathway P1 from CRHFT and UHDB, which is 4,861.
CHC domiciliary care as at 3 January 2023: 975 patients.
d. How many care staff are employed/contracted to deliver the service?
The framework is zero activity, zero cost. We do not hold information regarding how many staff are employed by the providers.
