Get Involved with Joined Up Care Derbyshire

Joined Up Care Derbyshire is a partnership of health and social care organisations in Derby and Derbyshire that aims to provide better, more integrated care for local people. One of the ways that they do this is by getting involved in various activities and projects that support their vision and values. Getting involved is a great way to make a difference, learn new skills, meet new people and have fun.

There are many opportunities for engagement, such as becoming a patient or carer representative, joining a focus group, taking part in surveys or events, or sharing your story or feedback. If you are interested in getting involved with Joined Up Care Derbyshire please fill in the form.

What opportunities are there to get involved?

Readers’ Panel

This panel reviews new and revised information that is going out to patients and members of the public, to make sure the information is understandable, clear, and concise. Find out more about this role.

Patient Participation Group Network

Patient Participation Groups represent the patient population of GP practices and are generally made up of a group of volunteer patients, the practice manager, and one or more GPs. They meet to discuss the services on offer, and how improvements can be made for the benefit of patients and the practice. Find out more about this role.

Patient and Public Partners

Patient and Public Partners are lay members who want to be involved in improving health and care. They have extensive experience either as a patient, family member or caregiver; others have been part of the health system in a professional manner.

Our partners get involved in various aspects of work in the Integrated Care System to help develop and improve services. Find out more about this role.
Who are we looking for?
We would like to invite people to get involved who have experience of health and care services or who are interested in the work they do. Depending on the work that we are doing, we may involve different people at different times.

We are looking for:

Members of local communities with an interest in health or care

Patients or previous patients

Carers, relatives and friends of patients

People from any age or background – there will also be opportunities for children and young people to get involved
