Right of Access to your Information (Subject Access Request)
The right of access, commonly referred to as a Subject Access Request (SAR), gives individuals the right to obtain a copy of their personal data, as well as other supplementary information, that is held by NHS Derby and Derbyshire ICB.
Please note that the Information Governance team can only deal with requests for information relating to the Integrated Care Board and is not responsible for providing information for hospitals or Local Authorities. If your query relates to one of the Joined Up Care Partner organisations, please refer to their web site for details of the process to be used to access medical records or other information.
