High Peak patient transport service reaches milestone 1,000 patient discharges
A patient transport service based in the High Peak has successfully completed 1,000 patient discharges.
Connex Community Support charity delivers a 6 day a week home from hospital transport service, safely returning patients and helping them settle back at home.
The service works alongside local community organisations to ensure patients have the right care package available when they return home.
Working with befriending services and volunteer transport, the comprehensive package ensures patients have access to the care that will keep them from being re-admitted to hospital.
Howard Gunn, Head of Transport Services at Connex, said: “The success of this service is down to our friendly delivery model, and the community connections we work hard to build.
“We work closely with the hospital to ensure arrival times are reliable and that care packages are in place at a patient’s home.
“Reaching 1,000 discharges is a significant milestone for us and we’re really proud to deliver such a friendly and local service that makes a difference.”
Colin, who was the 1000th patient, said: “This is such a good service and means that I’m able to get home to my family as soon as possible. After almost a week in hospital, it’s great to get out and home again.”
David Lilley-Brown, Partnerships Manager at NHS Derby and Derbyshire Integrated Care Board, said: “Reaching 1,000 discharges is a real achievement and reflects the value of working with trusted VCSE partners who know their communities well. Every one of those journeys represents a person getting back home safely, with the support they need in place.
“The patient transport service Connex provide plays an important role in easing pressure on hospital capacity. By reducing delays, supporting safe discharge and helping people settle back home they help to prevent avoidable readmissions which in turn helps ensure hospitals can focus on patients with the greatest clinical needs.
