Bringing the care transfer hub to life 

Getting people home safely and at the right time is a shared priority across Derbyshire’s health and care system and the care transfer hub is playing a key role in making that happen.

In the new video, Adam Hayward, care transfer hub operational lead at Derbyshire Community Health Services, explains what the hub is, how it works and why it matters.

As a joined-up team coordinating discharges across acute, community and social care, the hub is helping to reduce delays, improve flow and keep ‘home first’ at the centre of every decision. By bringing colleagues together through a single triage point and shared view of system pressures, the hub supports better and faster decision-making helping more people get home sooner and improving experiences for both patients and staff.
