
There are five Primary Care Networks, made up from over two dozen General Practices working together with community, physical and mental health, social care, pharmacy, hospital, and voluntary, charitable, faith and community services in local areas across it. Derby has some well-loved and vibrant Neighbourhoods, that have nevertheless scored very poorly when looking at economic deprivation and health inequalities. Around one in three residents live in an area that is among the top 20% most deprived in the country. 62% of residents over 18 are considered overweight or obese.
Other challenges Derby faces are lower than nationwide average life-expectancies and an ageing and increasingly unwell population.
Made up from a group of dedicated and knowledgeable stakeholder partners, from across the entire ‘system’, Derby Place Partnership Board have been focussing on supporting each other’s tangible actions to overcome these. Its aim is always to help residents to stay well for longer. Issues such as frailty, falls, childhood obesity and end of life care are always on the table. Over the years, new partnerships have been formed, specifically focussing on health inequalities and particular segments in Derby’s diverse communities.
