Chesterfield

As one of Derbyshire’s eight Neighbourhood Alliances, we bring together professionals from health, social care, mental health, local authorities, and the voluntary and community sector, alongside local residents, to reflect the real priorities and needs of Chesterfield people. We are passionate about building an empowered, resilient community and committed to delivering joined up, person centred care so residents can stay healthy and independent for longer.

Chesterfield has a population of approximately 103,569, with 49% male and 51% female residents. Around 21% are aged 65 or over, 60% are aged 18–64, and 19% are aged 0–17, reflecting a community with a broad and diverse range of health and wellbeing needs.

Our programme of work is focused on transforming services for people living with frailty (Ageing Well Programme) and addressing health inequalities using the CORE20PLUS5 neighbourhood approach through creating a community hub model such as the current Barrow Hill Memorial Hall hub development. This hub is envisioned as a ‘one-stop shop’ for local services, supporting generational change and improving access to care.

As part of this, we are striving for a joined-up approach to community engagement and service delivery, including integrating social prescribing offers, developing local navigation hubs, and leveraging community assets to deliver a ‘no wrong door’ approach. Insight from local communities continues to inform these changes, with quality conversations and engagement frameworks shaping how we enhance what is currently on offer. Our goal remains clear: to focus on what matters most to individuals and use their voices to guide meaningful improvements.
