Healthier You – NHS Diabetes Prevention Programme

Reduce your risk of developing type 2 diabetes by joining our programme. Make simple changes to your lifestyle and get support to eat better, move more and lose weight. Ask your GP or Practice Nurse to refer you today.

How can Healthier You help me to reduce my risk of developing type 2 diabetes?

Join our 9-month NHS Diabetes Prevention Programme (DPP) to start reversing the risks today.

  • Get guidance from experienced experts.
  • Benefit from 1-to-1 coaching.
  • Make the most of group support.
  • Receive personalised advice on nutrition, movement and motivation.

Choose from:

In person group programme

Join our in person group programme and enjoy fortnightly sessions held at a venue near you. Sessions are held at community centres, leisure centres or primary care practices across the county. Make friends, learn more about healthy living and help each other to stay motivated and reduce your risk together.

Digital programme

Join our digital-only programme and use an online app from Second Nature to start reducing your risk. Sign in online each week to complete the programme, with optional calls from a health mentor if needed.

Remote group programme

Complete our programme via regular online video calls (over Zoom) or telephone calls. This programme is designed to support those with visual/audio impairments, women with a history of gestational diabetes, and non-English speakers (suitable for Urdu, Punjabi and Bengali speakers).

What if I’ve had gestational diabetes?

Having gestational diabetes mellitus (GDM) raises your risk of developing type 2 diabetes. Want to find out how to lower your risk, or noticing changes in your body, health and wellbeing like might be hormone-related (like weight gain around your waist)?

Join our supportive programme designed especially for women with a history of GDM and find out more about female health to help you feel empowered, lose weight and reduce your risk of developing type 2 diabetes.

Is the Healthier You programme right for me?

To be eligible to join you must be aged 18-80 * and:

  • Have a blood test result in the last 12 months with a reading in the following range:
    • HbA1c: 42-47.9 mmol/mol
    • Fasting Plasma Glucose (FPG): 5.5-6.9 mmol/l

*If you’re over 80 you can still join the programme if you’re fit and able to engage with the programme. Please obtain a referral from your GP.

How much does the programme cost?

Nothing! It’s free.

How long is the programme?

The programme lasts 43 weeks (approx. 9 months) and includes an initial welcome call followed by 13 (2hr) group sessions.

How do I join a programme?

  • Check to see if you’re at risk of developing type 2 diabetes by visiting https://riskscore.diabetes.org.uk/start. If your risk score is over 16, contact your GP to arrange a blood test to confirm your risk. If your blood test results confirm your elevated risk, complete our registration form and one of our team will contact you to get started.
  • If you have had gestational diabetes (GDM) you don’t need a referral from your GP, simply complete our registration form to get started.

Diabetes Remission Programme

The NHS Type 2 Diabetes Path to Remission Programme

The NHS Type 2 Diabetes Path to Remission Programme is a free programme designed to help you reduce your diabetes medications, lose weight, improve your health, and potentially put your type 2 diabetes into remission.

You will be offered low calorie, total diet replacement products (typically soups, shakes and porridges) totalling 800‑900 kilocalories per day, to replace all normal meals for 12 weeks.  You will receive support and monitoring for 12 months, including help to re-introduce food after the initial 12-week period. 

We understand that everyone has their own preferences, and we provide two options for accessing the service to best suit your needs.

This will be either; 

  • One-to-one in-person support; or 
  • One-to-one digital support

Coaching sessions are delivered by one of our qualified diabetes practitioners, along with online learning and resources, including meal plans and recipes. This support will provide you with the help and advice you need throughout every phase of the programme. Your GP practice will be kept informed of your progress and if any medication changes might be needed.

How do I access the programme?

To be eligible you must;

• Be aged 18-65 years

• Have had type 2 diabetes first diagnosed within the last 6 years

• Have a BMI of 27kg/m2 or higher if you’re from a white ethnic background (or 25kg/m2 or higher if you’re from any other ethnic group)
