
We work hard to maintain a vibrant, lively, and inclusive Neighbourhood Alliance. Our representation is broad and encompasses the interests of the entire population of South Derbyshire, as we’ve built and expanded upon strong existing local strategic partnerships. We focus on the community at large; physical and mental health, adult and children’s social care, pharmacies, hospitals, and voluntary services involving, charities and faith organisations. Everyone in local our area is working together to improve the health of our local population, working with their strengths to do so.
Our Neighbourhood can evidence how they have been gaining traction at scale, across all members’ organisations on agreed and shared goals. We are courageous and collaborative. South Derbyshire Neighbourhood benefits from a group with demonstrable resilience, able to respond to the challenges posed in the local and regional economy and health and well-being settings. We have been able to gain some tangible successes, by working as integrated neighbourhood teams. This is especially important as within our area we face some of the highest levels of economic deprivation and health inequalities within the county and of the greatest concentration, contrasting with some of the most affluent small areas too.
The total population served is around 116,000, concentrated around Swadlincote. With the highest growth in Derbyshire and one of the most rapidly expanding populations, outside of the south east of England. A significant portion of our residents are now aged 65 years or over. Therefore, we have an ageing population, in common with other Derby and Derbyshire Neighbourhoods. However, expert population projections for South Derbyshire suggest that we will see an estimated 86% increase in over 65-year-olds by 2039. This will have a definite effect on the health economy of our area. We are working with our communities to address those health inequalities present, following a population health management approach. Applying every skill and resource available to us.
One of the key aims for Joined Up Care Derbyshire is to Live Well, for Longer. This ageing population presents a challenge to our community and one that the Alliance seeks to tackle collaboratively. Our Swadlincote PCN, made up from 5 GPs surgeries have been delivering an enhanced Home Visiting Service responding to the needs of housebound residents, many of whom are older, living at home and experiencing ill-health. Piloting this first to learn what works well, before implementing this more fully. We are all rising to the challenges to be faced together.
