1. How are Children & Young People (CYP) services commissioned for those who have complex health needs in your area? (as defined within Background). Please answer against the categories listed in the below, where applicable.
2. Please complete as much of the table within the attached spreadsheet as possible, contained on the 2nd tab. We would like to know the names of the care providers that are currently being used to deliver services to the Children & Young People (CYP) cohort described in the Background. To support with identifying a national list of care providers for this cohort, we also require details about the type of provider. Reference points such as the provider postcode and CQC Location ID (if it is a CQC registered service) have been requested to allow for the providers to be identified in a consistent and accurate manner across ICBs/ICSs.

The information included for Q2 is for the providers currently used to provide CYP continuing care packages, with a total of 16 providers.
Please note; out of the cohort of CYP patients used in the response, we have excluded 40 other packages as below:
• 26 of these being with a local authority and funded as direct payments for the employ of personal assistants:
o 2 with Derby City Council
o 24 with Derbyshire County Council
• 14 having a PHB Direct Payment package:
o It is not possible to identify these providers, as the individual families choose which services they want and when they want them.
We would suggest that you may wish to direct this request to the local authorities for more information on the packages that they fund
