Please may I request information under the FOI of details of who to contact/email who run or facilitate the NHS Health Check program (40-74 year old screening). I would welcome the information on which ICB (CCG) offer the service at surgery/pharmacy or independent contractor level

Thank you for your request for information below. I can confirm that the ICB do not hold the information that you are seeking as Health Checks fall under the remit of the Public Health Teams at the Local Authorities, although they are delivered through GP practices.
