The NHS Excellence Awards took place at the recent ConfedExpo, bringing together health care leaders from across the country.

Team Up were nominated and recognised for their innovative approach to proactive and reactive neighbourhood care. The team are a core part of the local neighbourhood model, and have transformed care for people with complex needs, improved outcomes and strengthened neighbourhood resilience.

Over the last year, Team Up partners delivered over 9,000 two hour responses and 31,000 same day urgent activities and services now complete over 7,000 home visits each month, releasing around 3,500 hours of GP time.

Reflecting on the award ceremony, Dr Penny Blackwell, Chair and Clinical Director for Neighbourhood Health and Care said: ” What really stood out at ConfedExpo was the quality of neighbourhood work happening across the country.

“The room was full of people who are driving change from the ground up, often in challenging circumstances, and doing it because they believe in the value of strong, connected communities.

“I’m proud of what this team has achieved, of the daily impact of this work, and the acknowledgement on a national platform, which only encourages me to forge ahead and develop further.”

For the Team Up team, the recognition has made a huge impact and validated the commitment to working with our communities to deliver better health outcomes.

Being recognised as a Regional Champion is a significant achievement and reflects the outstanding work taking place across the NHS to improve care, experience and outcomes for patients, communities, and colleagues.
