Integrated Care Systems are where health, social care, the voluntary sector and other partners come together. 

Integrated Care Systems have four main functions:

  • improving outcomes in population health and healthcare;
  • tackling inequalities in outcomes, experience and access;
  • enhancing productivity and value for money;
  • supporting broader social and economic development.

Our Integrated Care System is known as Joined Up Care Derbyshire.

Our priority is to make improvements to the Derby and Derbyshire populations’ life expectancy and healthy life expectancy levels in comparison to other parts of the country, and reduce the health inequalities that are driving these differences.

Our health, social care and voluntary sector organisations work together formally through our Integrated Care Partnership.
